Fertilisation rate is not the same as pregnancy rate. ICSI reliably solves a fertilisation problem, and the rest of the cycle then depends on egg quality, embryo development and the uterine lining exactly as in standard IVF.
What it costs

What actually happens, in order
Nothing here should be a surprise on the day.
The same start as IVF
Consultation call, testing, stimulation, monitoring scans, trigger and egg retrieval proceed exactly as in a standard IVF cycle.
Sperm preparation and selection
The sample is washed and prepared. The embryologist then examines individual sperm at high magnification and selects on movement and shape.
Egg preparation
The cells surrounding each egg are removed so maturity can be confirmed. Only mature eggs can be injected, which is why the number injected is often lower than the number retrieved.
The injection
Under a micromanipulator, a fine glass needle holds the egg steady and delivers one sperm into the cytoplasm. Each egg is done individually, by hand.
Fertilisation check
Sixteen to eighteen hours later we confirm which eggs fertilised normally. You get that report the next morning.
Culture and transfer
Embryos are cultured to day three or day five and transferred, or vitrified for a later cycle. Identical to IVF from here.

The factors that genuinely move the result
These are the things worth your attention. Most of what circulates online is not.
Egg maturity
Only mature eggs can be injected. This is why the retrieval number and the injection number differ, and we always explain both rather than quoting the flattering one.
Sperm DNA integrity
A sperm can look and swim perfectly and still carry fragmented DNA. Where miscarriage or failed fertilisation has happened before, testing this changes what we do next.
Embryologist skill
ICSI is a manual technique performed one egg at a time. This is the honest reason laboratory staffing matters more than laboratory branding.
The female side, still
ICSI solves a sperm problem. It does not improve egg quality or the uterine lining, so the rest of the assessment matters exactly as much as it does in IVF.
The part of ICSI that usually gets glossed over
- ICSI improves fertilisation. It does not improve egg quality, embryo quality or implantation, so it does not raise the success rate for couples who did not need it.
- We use ICSI where there is a clinical reason, and we write that reason in your file. It is charged as a separate line so you can see exactly what it adds.
- Some causes of severe male infertility are genetic and can be passed to a son. Where that applies we recommend genetic counselling before treatment, not after.

What people ask us about ICSI
Yes. IVF with ICSI starts at ₹1,40,000 against ₹1,30,000 for a standard IVF cycle. It appears as a separate line on your estimate so you can see exactly what it adds, and we will tell you if we do not think you need it.
Large studies show broadly similar outcomes. There is a slightly higher rate of certain congenital differences in ICSI pregnancies, generally attributed to the underlying infertility rather than the technique itself. We go through this in the consultation.
Azoospermia needs a proper andrology assessment first, because obstructive and non obstructive causes are treated differently. Where sperm can be retrieved surgically through TESA or PESA, it can be used for ICSI in the same cycle.
IMSI selects sperm at much higher magnification than standard ICSI. It is available here and can help in specific situations such as repeated ICSI failure or high DNA fragmentation. We recommend it only where there is a reason, not as a routine upgrade.
