Who it is for
- You have been trying for a year or more, whatever the female assessment shows
- A previous semen analysis was abnormal, or was never repeated
- There is a history of undescended testis, mumps, injury or hernia surgery
- You have had chemotherapy, radiotherapy or long term steroid use
- There is a varicocele, or swelling and discomfort in the scrotum

Most male factor infertility is workable
A low count is almost never the end of the road. ICSI needs very few sperm, and even where none are present in the ejaculate, surgical retrieval succeeds in many cases. What matters is finding out which situation you are in.
What it costs
A semen analysis is inexpensive and is quoted before you book it. Where treatment is needed, whether medication, varicocele repair or ICSI, you receive a full written estimate before anything is scheduled.
What actually happens, in order
Nothing here should be a surprise on the day.
Semen analysis, properly done
Count, motility, morphology, volume and viscosity, assessed to WHO criteria. Two samples a few weeks apart, because a single result varies more than most people realise.
History and examination
Medical history, medication, occupational heat exposure, previous surgery and a physical examination, including checking for varicocele.
Hormone profile
FSH, LH, testosterone and prolactin where indicated. A hormonal cause is one of the more treatable ones, so it is worth ruling in or out early.
Further testing if needed
Scrotal ultrasound, DNA fragmentation index, or genetic testing including karyotype and Y chromosome microdeletion where the count is very low or absent.
Treatment
Depending on the cause: medication, varicocele repair, lifestyle and occupational changes, or moving directly to ICSI where that is the realistic route.
Surgical sperm retrieval, where required
TESA or PESA retrieves sperm directly from the testis or epididymis under local anaesthesia or short sedation, for use in an ICSI cycle.

The factors that genuinely move the result
These are the things worth your attention. Most of what circulates online is not.
Sperm DNA fragmentation
Sperm can look normal on a standard report and still carry damaged DNA. This matters for recurrent miscarriage and repeated failed cycles, and it is not part of a routine semen analysis.
Varicocele
Enlarged veins in the scrotum raise local temperature and are found in a meaningful share of infertile men. Repair can improve parameters, though not in every case, and we will be honest about the odds in yours.
Heat and occupation
Long driving hours, industrial heat, laptops on laps and frequent hot baths all measurably affect sperm production. These are among the few genuinely modifiable factors.
Time
Sperm production takes about 74 days. Any change you make needs roughly three months before it shows on a report, so we build that into the plan rather than retesting too early.
Things worth saying out loud
- An abnormal semen analysis is a medical finding, not a verdict on you. It is also frequently improvable, and where it is not, ICSI works around it.
- One bad report is not a diagnosis. Results vary with abstinence period, illness in the previous three months, and how the sample was collected and handled. We repeat before concluding anything.
- Most male fertility supplements sold online have thin evidence behind them. We will tell you which of the few have reasonable support and which are marketing.
- Where the count is zero, the distinction between obstructive and non obstructive causes changes everything about what happens next.

It can be repaired surgically, and in appropriately selected men this improves semen parameters. Selection is the important word. We will tell you honestly whether repair is likely to help in your case or whether going straight to ICSI is the better use of your time.
A small number of antioxidants have modest supporting evidence. Most products marketed for male fertility do not. We will name the ones worth considering and tell you plainly that the rest are not worth your money.
